摘要
Abstract
Objective To explore the necessity that clincal pharmacists carry out pharmaceutical care in intensive care unit.Methods Based on knowledge of pharmacy and therapeutic drug monitoring,clinical pharmacists did medication adjustment of vancomycin for a patient who had severe pneumonia.Results and conclusion According to the therapeutic drug monitoring and renal function monitoring,we did medication adjustment timely,avoiding the patient from suffering severe renal damage.It is necessary for clinical pharmacists to carry out pharmaceutical care in the treatment of critical patients in ICU and do research on individual therapy of vancomycin to formulate more reasonable therapy.关键词
