糖化血红蛋白和N末端脑钠肽前体与2型糖尿病合并冠心病患者心力衰竭的相关性研究
Correlations between Glycosylated Hemoglobin and N-terminal B-Type Natriuretic Peptide with Heart Failure in Patients with Type 2 Diabetes Mellitus Combined with Coronary Heart Disease

Abstract
Objective To investigate correlations between glycosylated hemoglobin ( HbA1c) and N-terminal B-type natriuretic peptide ( NT-proBNP) with heart failure in patients with type 2 diabetes mellitus complicated with coronary heart disease. Methods Clinical data of 204 patients with type 2 diabetes mellitus admitted during January 2014 and De-cember 2016 was retrospectively studied, and the patients were divided into type 2 diabetes mellitus group (control group 1, n=102) and type 2 diabetes mellitus combined with coronary heart disease and heart failure (study group, n=102) ac-cording to whether the result of coronary CT angiography was normal or not, and other 102 healthy people taking medical ex-aminations at the same period were selected as control group 2. HbA1c, NT-proBNP, blood lipid, blood glucose, blood pressure and related indexes of cardiac function were compared and analyzed among 3 groups, and correlations between HbA1c and NT-proBNP with end systolic volume (ESV), end diastolic volume (EDV) and ejection fraction (EF) of left ventricle were analyzed. Results There were significant differences in levels of HbA1c, NT-proBNP, blood lipid, blood glucose, blood pressure and related indexes of cardiac function between study group and control group 1 and control group 2 (P<0. 05). In study group, HbA1c, NT-proBNP, ESV and EDV levels were significantly higher, while EF level was significantly lower than those in control group 1 (P<0. 05). In study group, in patients with gradeⅣcardiac function, HbA1c, NT-proBNP, ESV and EDV levels were significantly higher, while EF level was significantly lower than those in patients with gradeⅡandⅢcardiac function (P<0. 05). HbA1c and NT-proBNP were positively correlated with ESV and EDV, while they were negatively correlated with EF (P<0. 001). Conclusion HbA1c and NT-proBNP levels are correlated with related indexes of cardiac function in patients with type 2 diabetes mellitus combined with coronary heart dis-ease, and therefore clinicians may evaluate pathogenetic conditions by detecting HbA1c and NT-proBNP levels.关键词
