振幅整合脑图对新生儿急性胆红素脑病早期脑损伤及预后的价值
Application value of amplitude-integrated electroencephalography in the prediction on early brain damage and prognosis in neonates with acute bilirubin encephalopathy

Abstract
Objective To explore the value of amplitude-integrated electroencephalographic (aEEG) in predicting the degree of early brain damage and prognosis in neonates with acute bilirubin encephalopath .Methods From January 2014 to January 2016 ,24 cases with acute bilirubin encephalopathy in our NICU were chosen .All children received aEEG monitoring ,and the re-sults of functional brain ,brainstem auditory evoked potential (BAEP) and the results of clinical prognosis were analyzed .Results The characteristics of aEEG tracings in these infants were shown continuous normal voltage (7 cases) ,discontinuous voltage (15 cases) ,burst-suppression (2 cases);10 infants had electrical seizures :single seizure (5 cases) ,repetitive seizures (1 case) ,and sta-tus epilepticus (4 cases);Sleep wake cycle (SWC) recording :mature SWC (5 cases) ,immature SWC (15 cases) ,no SWC (4 ca-ses) .The judgment of abnormal level :17 cases had mildly abnormal aEEG and 7 cases had severe abnormalities .For the result of 6 months follow-up ,there were 13 cases of aEEG returned to normal ,5 cases of mild abnormalities ,4 cases of severe abnormalities ,2 cases of death .The results of BAEP :4 cases of mild abnormalities ,9 cases of moderate abnormalities ,11 cases of severe abnormali-ties ,the abnormality degree of BAEP was positively correlation with acute aEEG .Developmental quotient (DQ) of these newborns were followed up for 6 months ,there was statistical difference in prognosis between neonates with mild aEEG and severe abnor-malities in acute stage(χ2 =13.008 ,P<0.001) .There were significant differences in the prognosis of neonates with SWC (χ2 =14.400 ,P=0.006) .The results of aEEG classification and sleep wake cycle were correlated with the prognosis of the neonates (r=0.734 ,0.525 ,P<0.01) .Conclusion aEEG is helpful for early identification in acute bilirubin brain damage and prognosis , can provide a reliable basis for early intervention .关键词
