经皮内镜下治疗腰椎间盘突出症的疗效观察

Abstract
Objective To investigate the clinical efficacy of percutaneous endoscopic treatment via transforaminal approach or interlaminar approach on the patients with the lumbar disc herniation.Methods Endoscopic microendoscopic excision of nucleus pulposus ( with PELD) and decompression were used to treat 32 patients with lumbar disc herniation , of which 27 cases were via transforaminal approach and 5 cases were via interlaminar approach.The level of back and waist pain were evaluated by visual analogue scale ( VAS) before and after operation , and the activity of waist after 6 months postoperation were evaluated by modified MacNab scores.Results All cases were followed up for 6~12 months.VAS:the average 27 cases of transforaminal approach were 9.19 ±0.74 preoperatively , and decreased to 1.37 ±0.69 at 1 month postoperatively , and 1.52 ±0.58 at 3 months postoperatively.There were significant differences with the VAS between 1, 3 months postoperation and the preoperation (P＜0.05).The average 5 cases of interlaminar approach were 9.40 ±0.55 preoperatively , and decreased to 1.00 ±0.71 at 1 month postoperatively and 1.20 ±0.84 at 3 months postoperatively.There were significant differences with the VAS between 1, 3 months postoperation and preoperation (P＜0.05).The excellent-good rate of modified MacNab scores at 6 months after operation were 94.5%.Conclusions PELD via transforaminal approach or interlaminar approach are effective to treat the lumbar disc herniation.关键词
