闭合和切开复位克氏针内固定治疗儿童肱骨髁上Gart-landⅡB、Ⅲ型骨折的疗效比较
Clinical efficacy of closed reduction and open reduction and Krischner wire fixation for the treatment of humeral supracondylar fracture with Gartland type ⅡB and type Ⅲ in children

Abstract
Objective To investigate the clinical efficacy of closed reduction and open reduction and Krischner wire fixation for humeral supracondylar fracture of Gartland type ⅡB and type Ⅲin children.Methods The 79 cases of supracondylar fracture of humerus were divided into two groups according to different treatment methods.The closed group (39 cases) underwent closed reduction and Kirschner wire fixation.The open group (40 cases) underwent open reduction and Kirschner wire fixation.The operation duration ,intraoperative blood loss , Baumann angle , the carrying angle and eldow function at 3 months after operation were compared between two groups.Results The average operation duration of closed group was (22.85 ±3.79)min and that was (34.22 ±2.78) min in open group, and there was statistically significant difference between two groups (P＜0.001).The intraoperative blood loss of closed group was (1.85 ±0.81)ml and that in open group was (13.12 ±1.85) ml, and there was statistically significant difference between two groups ( P＜0.001) ,there were no complications such as delayed union ,compartment syndrome or iatrogenic lesions to vessels and nerves.The 79 cases were followed up for 3~6 months.Two groups of 3 months of testing the carrying angle ,Baumann angle ,excellent-good rate of eldow function compararison had no significant differences(P＞0.05).Conclusions Clinical efficacy are similar that closed reduction and open reduction and Krischner wire fixation are used to treat humeral supracondylar fracture with Gartland type ⅡB and type Ⅲ in children,but closed reduction method is less trauma , the operation time is short ,less intraoperative blood loss ,and does not affect the appearance of the skin.关键词
