Abstract
Objective To compare the effects of different doses of oxycodone injection in patient-controlled analgesia after transurethral ureteroscopic lithotripsy.Methods A total of 210 patients who had received combined spinal and epidural analgesia transurethral ureteroscopic lithotripsy and required postoperative analgesia were selected and randomly divided into groups A,B and C (n=70,respectively).Patient-controlled analgesia pumps were applied in the three groups for postoperative analgesia,and the analgesic solution ratio was:oxycodone hydrochloride 50 mg+tropisetron 6 mg+0.9％ normal saline (NS),100ml,background dose 2 ml/h.The single dose in groups A,B and C was 1,1.5 and 2.0 mg respectively,and the locking time was 5 min.The VAS score,Ramsay score,adverse reactions and analgesic pump pressure times at different time points after the operation among the three groups were compared.Results The VAS scores four,eight,16 and 24 hours after the operation in groups B and C were significantly lower than those in group A,and the Ramsay scores four and eight hours after the operation were much higher than those in group A (P ＜ 0.05);within 48 hours after the operation,the effective pressure times in group A was more than that in groups B and C(P ＜ 0.05),and there was no statistical difference between groups B and C (P ＞ 0.05);within 48 hours after the operation,the incidence of dysphoria in groups B and C was much lower than that in group A (P ＜ 0.05);among the three groups,the incidence of nausea in group C was the highest,follow ed by that in group B,and that in group A was the lowest (P ＜ 0.05);there was no statistically difference in the incidence of emesis and respiratory depression among the three groups(P ＞ 0.05).Conclusion Oxycodone has good effects in patient-controlled analgesia after transurethral ureteroscopic lithotripsy,but analgesic pump with single pressure dosage of 1.5 mg has better controlled analgesia effects,fewer adverse reactions and safer clinical application.关键词
