顺铂粉针剂联合多西他赛注射液治疗中晚期宫颈癌的临床研究
Clinical trial of cisplatin injection combined with docetaxel injection in the treatment of medium-terminal cervical carcinoma

Abstract
Objective To observe the clinical efficacy and safety of different doses of cisplatin combined with docetaxel in the treatment of advanced cervical cancer.Methods A total of 180 patients with A stage ⅡB-ⅣA cervical cancer were randomly divided into the control group,low-dose group,medium-dose group and high-dose group,with 45 patients in each group.The control group was given docetaxel 70 mg · m-2,intravenous infusion,once a week;On the basis of the control group,the low-dose group was treated by cisplatin 20 mg · m-2,intravenous drip,once a week.The medium-dose group received 30 mg · m-2 cisplatin,intravenous drip,once a week.The high-dose group received 40 mg·m-2 cisplatin,intravenous drip,once a week.All groups were treated for 6 weeks.The clinical efficacy and the incidences of adverse drug reactions were compared between the groups.Results The total effective rates of the low-,medium-and high-dose group and the control group were 60％ (27 cases/45 cases),53.33％ (24 cases/45 cases),40％ (18 cases/45 cases) and 33.33％ (15 cases/45 cases) respectively.The 3 year survival rates of the low-,medium-and high-dose groups and the control group were 64.44％ (29 cases/45 cases),84.44％ (36 cases/45 cases),80％ (38 cases/45 cases) and 51.11％ (23 cases/45 cases),respectively.The adverse drug reactions in low,medium and high-dose group included abnormal stomach discomfort,while the control group appeared abnormal hemograms and allergic reactions.The incidences of adverse drug reactions in the low-,medium-and high-dose group and control group were 8.89％ (4 cases/45 cases),15.56％ (7 cases/45 cases),24.44％ (11 cases/45 cases) and 4.42％ (2 cases/45 cases),and there were significant differences between the medium-dose group and the high-dose group,as well as between the low-dose group and the control group (P ＜0.05).Conclusion Cisplatin combined with docetaxel in the treatment of advanced cervical cancer exerted considerable efficacy,however the incidence of adverse drug reactions and the 3-year mortality rate were increased,therefore the dosage should be determined according to patients' condition.关键词
