Characteristics of Cardiopulmonary Function during Exercise in Patients with Mediastinal Diseases

Abstract
Objective To explore the characteristics and possible associated factors of cardiopulmonary function during exercise in patients with mediastinal diseases. Methods This retrospective study was conducted in 76 participants meeting the inclusion criteria who received inpatient treatment in Beijing Chest Hospital,Capital Medical University from November 2014 to November 2016,including 44 cases of mediastinal diseases (case group),and 32 cases with normal resting electrocardiogram, normal resting pulmonary function and normal exercise electrocardiogram (control group). We divided the case group into vessel invasion subgroup (n = 34)and non-vessel invasion subgroup (n = 10)based on the vessels being invaded by the lesion or not. We divided the case group into anterior mediastinal subgroup (n = 34)and middle and back mediastinal subgroup (n = 10) based on the position of lesion. The rest electrocardiogram,resting pulmonary function and cardiopulmonary exercise test (CPET) were measured,and the data of these measurements in all the patients were recorded. Results The rest electrocardiogram of all the patients presented normal sinus rhythm without ischemic ST-segment and T wave changes. Vital capacity% predicted (VC%)in case group was smaller than that in control group,but ratio of residual volume to total lung capacity (RV/ TLC)and resistance at 5 Hz% predicted (R 5%)in case group were higher than those in control group (P < 0. 05). Compared with the control group,the case group had lower values of exercise Watt % predicted (W%),maximal oxygen uptake % predicted (V·O 2%),oxygen pulse % predicted (V·O 2 / HR%),expiratory tidal volume (VTex),ratio of tidal volume to vital capacity (VT/ VC)and transcutaneous oxygen saturation of hemoglobin (SpO 2 )but higher breathing frequency (BF) (P < 0. 05).W% and breathing reserve (BR)in the middle and back mediastinal subgroup were higher than those in anterior mediastinal subgroup (P < 0. 05). Lower dyspnea index (DI)was observed in the middle and back mediastinal subgroup rather than the anterior mediastinal subgroup (P < 0. 05). W% in the vessel invasion subgroup was lower than that in non-vessel invasion subgroup (P < 0. 05 ). Conclusion During CPET,cardiopulmonary function during exercise decreased in patients with mediastinal diseases,which may be related to the resting pulmonary function,the location of the lesion and the vessels invaded by lesion.关键词
