Association between urinary albumin/creatinine ratio and early neurological deterioration in patients with acute ischemic stroke

Abstract
Objective To investigate the relationship between urinary albumin /creatinine ratio ( UACR ) and early neurological deterioration ( END) in patients with acute ischemic stroke .Methods Two hundred and sixteen patients of acute ischemic stroke diagnosed by brain MRI and MRA were recruited between July 2014 and December 2016 in the department of Geriatrics and Neurology , the First Affiliated Hospital of Nanjing Medical University .The symptoms of neurological deficits were assessed repeatedly based on the NIHSS within 7 d after admission .Any score increased ≥2 points during the first 7 d after admission was defined as END .All of the objects were categorized into the END group or non-END group.Univariate analysis, multivariable Logistic regression analysis and the stratification analysis were performed to explore the relationship between UACR and END .Results Compared with the non-END group, alcohol use, NIHSS score and UACR level were significantly higher in the END group (all P<0.05).The results of logistic regression analysis revealed that UACR was an independent risk factor for END in patients with acute ischemic stroke (OR=4.54,95％CI:2.08-9.94,P<0.001).The stratification analysis probed that higher UACR level could better predict the development of END , especially in the patients older than 67 year-old, NIHSS at admission >3, accompanied with hypertension , with the TOAST subtype of large-artery atherosclerosis or male patients .Conclusions Increased UACR level is significantly associated with the development of END in patients with acute ischemic stroke .It has certain clinical value .关键词
