Abstract
Objective To evaluate the clinical relevance and prognostic value of immunophenotype and leukemiaassociated immunophenotype (LAIP) in acute myeloid leukemia (AML).Methods Ten-color multiparametric flow cytometry (FCM) with CD45/side scatter(SSC) gating was used to detect the expressions of the following antigens as exemplified as CD2,CD4,CD7,CD19,CD79a,CD11b,CD13,CD15,CD33,CD64,CD14,CD117,myeloperoxidase (MPO),human leukocyte antigen (HLA)-DR,CD34,CD38 and CD56 in 164 AML (non-M3) patients.Results In 164 AML patients,the most frequent myeloid-associated antigens were CD13 (97.6%),CD33 (97.6%),CD117 (89.6%),and MPO (84.8%),respectively.The antigens mainly expressed on the surface of hematopoietic stem/progenitor cells (HSC/HPC) were CD38 (92.7%),HLA-DR (84.8%),and CD34 (63.4%),respectively.Lymphoid-associated antigens,such as CD7 (30.5%),CD56 (22.0%),CD4(18.3%),and CD19(9.8%),respectively,were expressed in some AML patients.Among these antigens,CD19,CD56 were mostly expressed in M2,especially in M2b.Furthermore,it was found that CD19+ and/or CD56+ patients showed higher complete remission(CR) rate than others (P=0.048 and P=0.038,respectively).There were about 56.7% patients presenting definite LAIP,mostly in M2,M4,and M5.The most frequent aberrant phenotypes of LAIP were cross-lincage antigen expression (87.1%,mostly CD7+CD117+CD34+/-,CD56+CD117+CD34+/-),antigen dim/st expression (32.3%,mostly showing CD33dim),and asynchronous antigen expression (6.5%,mostly showing co-expression of CD11b and CD34).Among these aberrant type of LAIP,patients harboring asynchronous antigen expression showed shorter overall survival(OS) and disease-free survival(DFS) than the other types,which might predict poor prognosis.Conclusions Taking definite immunophenotyping and LAIP into account may benefit for predicting prognosis and guiding treatment.关键词
