头侧中间入路的腹腔镜直肠癌根治术:保留左结肠动脉的临床意义
Cephalo-medial to lateral laparoscopic total mesorectal excision with left colic artery preserving for rectal cancer: a retrospective study

Abstract
Objective To assess the safety,feasibility,radical resection,short term outcome and surgical technique of cephalo-medial to lateral approach in laparoscopic radical rectal resection with left colic artery preserving for rectal cancer.Methods Retrospective analysis was performed with rectal patients in our hospital from July 2016 to December 2016.There were 32 cases of laparoscopic radical rectal resection with left colic artery preserving using cephalo-medial to lateral approach as CML group and 29 cases with traditional-medial to lateral approach as TML group.We aimed to investigate the surgical technique by comparison between two groups.Results There was no case needing conversion to open,with intraoperative severe complications and mortality.The operative time was (103.6±16.9) min in CML group with (17.2±2.7) min of lymph node dissection time,(95.3±21.5) mL blood loss and (17.1±7.2) lymph node harvest,which were similar to those in TML group.No.253 lymph node harvest was (3.3±1.2),which was significantly more than that in TML group (1.9±1.2,P＜0.05).There was no statistical difference of postoperative morbidity,postoperative pass flatus time and hospital stay between two groups (P＞0.05).Conclusions Cephalo-medial to lateral approach was safe and feasible for laparoscopic radical rectal resection with left colic artery preserving of rectal cancer.The operative time of cephalo-medial to lateral approach was comparable to the conventional approach.It could provide better No.253 lymph node dissection during the surgery.Further study with larger sample is required to evaluate the impact of cephalo-medial to lateral approach for laparoscopic radical rectal resection with left colic artery preserving of rectal cancer.关键词
