Causes Analysis and Precautionary Measures of Medication Errors in Inpatient Pharmacy in Our Hospital

Abstract
Objective To reduce medication errors and improve the quality of drug dispensing, so as to ensure the medication safety of patients in inpatient pharmacy. Methods Totally 239 cases of medication errors in inpatient pharmacy in our hospital from 2013 to 2015 reported to Monitoring Network of Medication Safety in China Core Group of International Network for the Rational Use of Drugs (INRUD) were classified and analyzed. Results The common medication errors were drug dosage(23. 85％ ), frequency of drug adminis-tration(20. 08％ ) and varieties of drugs(20. 08％ ), among which the main mistakes were caused by primary pharmacists(83 cases) and primary doctors(47 cases). Conclusion The standardized management of drug in the hospital pharmacy and the establishment of a double check system can reduce the occurrence of medication errors.关键词
