Abstract
Objective:To observe the ultrasonographic features of upper gastrointestinal tract perforation and perforation site,and to investigate the diagnostic value of ultrasound in the upper gastrointestinal tract perforation and the causes of misdiagnosis.Methods:Sixty-seven cases of upper gastrointestinal tract perforation diagnosed by surgery and pathology were analyzed retrospectively.The direct and indirect signs of ultrasonography were observed,and the diagnostic accuracy was compared with X ray film.The causes of misdiagnosis were summarized.Results:Fifty-nine cases (88.06％) showed direct or indirect signs of the upper gastrointestinal tract perforation,53 cases (88.06％) showed free gas in the abdominal cavity,59 cases (61.19％) showed perforation site,62 cases (92.54％) showed abdominal cavity effusion,the diagnostic accuracy was 92.54 ％.Three cases (4.48 ％) of escaped diagnosis,2 cases (2.99％) of misdiagnosis.X-ray films showed 48 cases (71.46％) of free gas in the abdominal cavity,7 (10.45％) cases of abdominal cavity effusion,the diagnostic accuracy was 71.64％,the difference between the two methods was statistically significant (P ＜ 0.05).Conclusion:Ultrasound can directly display the location of perforation and the free gas in the abdominal cavity.Combined with indirect signs,it has higher diagnostic value for upper gastrointestinal perforation.The main cause of escaped diagnosis or misdiagnosis of upper gastrointestinal tract perforation are the doctor lack of clinical experience and operate skill not appropriate,the interference of gastrointestinal gas,the complexity and diversity of the patients condition.关键词
