妊娠期瘢痕子宫破裂11例病例分析及文献复习

Abstract
Objective:To analyze the clinical characteristics,risk factors and prevention of the scar uterine rupture during pregnancy.Methods:The clinical features of 11 patients with complete uterine rupture during pregnancy in Peking Union Medical College Hospital from January 2000 to June 2016 were retrospectively analyzed.According to the previous uterine surgery type,all the patients were divided into caesarean section group (n=4) and gynecologic operation group (n=7).Results:Among 11 patients with scar uterus,four patients (36.4％) had selective caesarean without uterine contraction;seven patients (63.6％) had myomectomy;five patients had myomectomy once,1patient had twice myomectomy and 1 patient had the history of different parts of uterine surgery (myomectomy and hysteroscopic resection for endometrial polyp).In present pregnancy,uterine rupture occurred at an average of (23.8±6.5) gestational weeks in the patients with caesarean,while (34.6±5.2)gestational weeks in the patients with myomectomy.Maternal-fetal prognosis:10 pregnancy women survived;1 patient had uncontrolled hemorrhagic shock followed with ischemia anoxic encephalopathy and died.Five newborn were alive healthy;five fetuses had been dead at the time of treatment;one newborn had severe asphyxia complicated with ischemic anoxic encephalopathy and died.Conclusions:Surgeons should suture cautiously during gynecology surgery or caesarean section to prevent the uterine scar defect.Evaluation of the uterine scar should be done before pregnancy.It is necessary to beware of uterine rupture during the whole pregnancy and improve the outcome of pregnancy.关键词
