胰岛素样生长因子-1与促排卵结局及抑制素B、血管内皮生长因子的相关研究
Correlations between IGF-1 and inhibin B, VEGF & controlled ovarian hyperstimulation outcome

Abstract
Objective:To explore insulin-like growth factor-1 (IGF-1) level in serum and follicular fluid (FF)during different controlled ovarian hyperstimulation (COH) protocols,and the association between IGF-1 and inhibin B (INHB),vascular endothelial growth factor (VEGF) & COH outcomes.Methods:The data of 76 patients who underwent IVF/ICSI for the first time at Bethune International Peace Hospital from July to December 2015 were retrospectively analyzed.According to different ovulation induction protocols,the patients were divided into down-regulation protocol group (DRP group,n=38) and non down-regulation protocol group (NDRP group,n=38).All the serum and FF samples in COH were collected,and the double antibody sandwich enzyme-linked immunosorbent assay (ELISA) was used to detect the serum and FF levels of IGF-1,INHB,and VEGF.The correlations among IGF-1 with INHB,VEGF and COH outcomes were analyzed.Results:There were no significantly differences in the basal serum IGF-1 between the two groups.The serum IGF-1 was significantly decreased after down-regulation,and IGF-1 in DRP group on gonadotrophin (Gn) starting day was significantly lower than that in NDRP group [(194.30±43.50) vs.(223.46±67.03) ng/ml] (P＜ 0.05).There was no significant difference in IGF-1 levels after Gn stimulation between the two protocols (P＞0.05).However,the IGF-1 levels in serum showed an upward trend,and reached the peak on the day after HCG day and then declined in the both group (P＜0.05).In addition,IGF-1 levels on the fifth day of stimulation had the best correlation with COH outcomes (P＜0.01).The levels of IGF-1 in FF were increased with the follicles enlargement in both groups and IGF-1 levels in big follicles were significantly positively correlated with COH outcomes.There were no significant difference in IGF-1 levels between the large and medium follicles (P＞0.05).However,the IGF-1 levels in small follicles of NDRP group were significantly higher than those in DRP group [(155.14±27.59) vs.(134.72±40.53) ng/ml] (P＜0.05).Besides,the IGF-1 levels in serum and FF were positively correlated with INHB levels (P＜0.01),but negatively correlated with VEGF levels (P＜0.05).Conclusions:The decline of serum IGF-1 levels after down-regulation may be conducive to follicular synchronization.During COH process,IGF-1 levels in serum and FF of large & medium follicles are positively correlated with COH outcome.IGF-1 may promote INHB secretion and inhibit VEGF secretion.关键词
