慢速冷冻卵裂期胚胎复苏后培养至囊胚移植的临床结局分析
Clinical outcome of slow frozen-thawed cleavage stage embryos transfer after extended culture to blastocysts

Abstract
Objective:To analyze the clinical outcome of slow frozen-thawed cleavage stage embryos transfer after extended culture to blastocyst.Methods:The data of slow frozen-thawed cycles with Day 3 cleavage embryos from January 2013 to December 2016 were retrospectively analyzed.All the frozen-thawed embryos of each couple were from one oocyte retrieval cycle.All the cycles were divided into two groups according to transplant embryo strategy:the cleavage embryos were directly transferred after slowly frozen-thawed in the cleavage transplant group (CTG group,n=55);the slow frozen-thawed D3 cleavage stage embryos were transferred after extended culture to blastocyst in the blastocyst transfer group (BTG group,n-=57).The blastocyst formation rate in BTG group was calculated.The implantation rate,clinical pregnancy rate,ectopic pregnancy rate,multiple pregnancy rate,early abortion rate and live birth rate were compared between the two groups.Results:The survival rate of slow frozen-thawed Day3 cleavage stage embryos was 72.57% and the blastocyst formation rate of extended culture after thawed was 29.50%.The average number of embryos transferred in CTG group was significantly higher than BTG group [(2.64±0.53) vs.(1.39±0.54),P<0.05],while the percentage of top-quality embryos transferred in both groups had no statistic difference (58.82% vs.47.37%,P>0.05).The clinical pregnancy rate (44.44% vs.39.02%),ectopic pregnancy rate (5.00% vs.0),multiple pregnancy rate (20.00% vs.25.00%),early abortion rate (20.00% vs.18.75%)and live birth rate (26.19% vs.28.21%) were not significantly different between two groups(P>0.05).The implantation rate (22.69% vs.33.33%) of CTG group was slightly decreased,but was not significantly different compared with BTG group (P>0.05).Conclusions:The clinical outcome of two groups was comparable.The blastocyst transfer after extended culture significantly reduces tle number of embryos transferred,and also increases the risk of no available blastocyst formation.So the appropriate slow frozen-thawed embryo transfer strategy should be chosen based on the patients' intentions and specific circumstance carefully.关键词
