Analysis of the etiology,risk factors and hematoma expansion in intracerebral hemorrhage

摘要

Abstract

Objective To discuss the risk factors of hematoma expansion in early stage of intracerebral hemorrhage. Methods Retrospective analysis the clinical data of 138 cases with acute hypertensive intracerebral hemorrhage,compare the differences between patients with hematoma enlargement and those who didn' t. Results Fifty patients developed he-matoma enlargement. The results of univariate analysis showed that the onset time to the first line of head CT,admission GCS score,admission NIHSS score and serum PT,APTT,TG,HDL-C,APO-A1,blood glucose and Cys C levels were statisti-cally significant among two groups(P<0. 05). Multivariate logistic regression analyses show that a shorter Onset-to-Scan Time(OR=1. 374,95%CI=1. 044~7. 666,P<0. 05),low admission GCS(OR=0. 222,95%CI=0. 092~0. 533,P<0. 01),high admission NIHSS score(OR=1. 735,95%CI= 1. 762~2. 624,P<0. 01),high serum levels of HDL-C(OR=1.726,95%CI=1.546~2.313,P<0.05)and blood glucose(OR=1.410,95%CI=1.027~1.934,P<0.05) were independent risk factors for early hematoma expansion in intracerebral hemorrhage. Conclusion The risk factors for early hematoma expansion in acute intracerebral hemorrhage are low admission GCS score,high admission NIHSS score,high ser-um level of HDL-C and blood glucose,as soon as possible after the onset of cerebral hemorrhage head CT examination can increase the detection rate of hematoma expansion.
