Diagnosis and Treatment of Bronchial Pleomorphic Adenoma:a Case Report and Literature Review

Abstract
We reported a case of pleomorphic adenoma of the right main bronchus who was diagnosed by electronic bronchoscopy and was cured by interventional bronchoscopic treatment in February 2015,the Affiliated Hospital of Southwest Medical University. We searched the databases of WANFANG DATA, CNKI, and VIP via using the keyword"支气管多形性腺瘤", and searched PubMed by using "polymorphic adenoma,bronchial" as the keywords, for related studies collected from the time of database establishment to December 2016. The included studies were involved in retrospective analysis.Results Diagnosis and treatment of this 45-year-old male case of pleomorphic adenoma of the right main bronchus: he manifested with cough, expectoration, and exertional dyspnea. Chest CT revealed round soft tissue masses in the right main bronchus. Pathological and immunohistochemical examination of electronic bronchoscopic biopsy specimen confirmed the diagnosis. After receiving bronchoscopic cryotherapy and argon plasma coagulation for removingthe tumor under general anesthesia with endotracheal intubation, his clinical symptoms disappeared. Postoperative examination made sure that the right main bronchial lumen was unobstructed, and no recurrence was found during 2 years of follow-up. Fifteen related articles were retrieved in which 16 cases of bronchial pleomorphic adenoma were reported. Taken our case into account, there were 17 cases totally, including 10 males and 7 females, and their age ranged from 11 to 76. The main symptoms included cough, dyspnea, fever, expectoration, hemoptysis. The main imaging findings were endobronchial mass and atelectasis. Bronchoscopic examination was performed in 14 cases. Nine cases underwent thoracic surgical treatment, and 8 cases underwent interventional therapy by flexible bronchoscopy, including 6 cases via high-frequency electric cautery, 4 cases via cryotherapy and 3 cases via argon plasma coagulation. Twelve patients had no postoperative recurrence in the follow-up.Conclusion The bronchial pleomorphic adenoma is a rare benign central airway tumor with non-specific clinical symptoms and image features. It can be confirmed by pathological and immunohistochemical examination of the flexible bronchoscopy biopsy. Interventional bronchoscopic therapy is an effective way to treat bronchial pleomorphic adenoma.关键词
