心力衰竭患者症状群与生活质量的相关性研究
Relationship between Symptom Cluster and Quality of Life in Patients with Heart Failure

Abstract
Objective To investigate the symptoms of patients with heart failure, and to explore the relationships between symptom clusters and the quality of life. Methods A total of 186 hospitalized patients with heart failure in one calss Ⅲ grade Ⅰhospital in Jinan were selected by convenient sampling method and investigated by using the Chinese version of Memorial Symptom Assessment Scale-heart Failure and the Minnesota Living with Heart Failure Questionnaire. ResuIts Patients demonstrated multiple symptoms in the course of disease development and the most prevalent symptoms were shortness of breath,dyspnea and fatigue when lying on their back.Through principal component analysis,5 symptom clusters were extracted including respiratory symptom cluster,fatigue symptom cluster, digestive tract symptom cluster, ischemic symptom cluster and congestion symptom cluster. The scores of quality of life were positively correlated with the total scores of the 5 symptom clusters. Multiple linear regression analysis revealed that respiratory symptoms,fatigue symptoms and congestion symptoms were the influencing factors of quality of life(P<0.001). ConcIusion There are multiple symptoms clusters in patients with heart failure and they seriously affect patients' quality of life. In order to relieve symptoms and improve the quality of life,medical staff should pay attention to the symptom management and control in a symptom cluster perspective.关键词
