Abstract
Objective To investigate causes for patients to seek diagnosis and treatment for BPPV repeatedly from different providers in different hospitals.Methods The clinical characteristics of 105 patients who sought for diagnosis and treatment for BPPV repeatedly from December 2012 to December 2015 were reviewed.Results Of these 105 pa-tients,23 were male and 82 were female,with ages ranging from 38 to 80 years(mean=61 years).Medical examinations in these patients included head CT/MRI,cervical spine X-ray/CT/MRI and cerebrovascular Doppler tests.Underlying or comorbid diseases were identified in 79 patients,including cervical spondylosis (n=30),hypertension/diabetes/lacunar infarction (n=19),hypertension/lacunar infarction (10),chronic otitis media (n=9),Meniere disease (n=6) and sudden deafness(n=5).Fifty four patients(51.4%)were admitted,25 were treated with i.v.transfusion and oral medications on an outpatient basis,and 26 received repositioning maneuvers in the outpatient clinic.Previous diagnoses included cervi-cal vertigo(n=29),cerebral infarction(n=17),Meniere's disease(n=5),vertigo syndrome(n=28)and refractory BPPV not responding to repositioning maneuvers(n=26).Final diagnoses included right posterior semicircular canal BPPV(n=59),left posterior semicircular canal BPPV(n=36),right horizontal semicircular canal BPPV(n=5),left horizontal semi-circular canal BPPV(n=3)and left inferior semicircular canal BPPV(n=2).In 37 cases(35.2%),positioning tests need-ed to be repeated twice or more to induce typical nystagmus.No typical nystagmus could be induced in 8 cases,al-though their symptoms resolved after repositioning maneuvers.Conclusion For BPPV patients,seeking medical care re-peatedly may be related to the underlying disease,patient-physician rapport and repositioning maneuver skills on the part of the physician.关键词
