咽鼓管球囊扩张术联合鼓膜置管治疗难治性分泌性中耳炎的分析

Abstract
Objective To compare clinical outcomes and safety of balloon dilation eustachian tuboplasty com-bined with tympanotomy tube insertion in the treatment of refractory secretory otitis media under either local or general anesthesia.Methods A total of 40 patients(48 ears)with refractory secretory otitis media were enrolled from December 2015 to November 2016 in this study.They were randomly selected into to receive local anesthesia monitored by an an-esthesia care group or general anesthesia(20 each).Average hearing threshold,EDTQ-7 results,intraoperative and post-operative events and surgery costs were compared between the two groups.Results Intraoperative awareness and pain re-action were reported with local anesthesia under monitored anesthesia care,although there was no significant difference in EDTQ-7 results,intraoperative blood loss,anesthesia effects at 6 months followed up or postoperative VAS scores be-tween the groups(P>0.05).Operative time,postoperative anesthesia events and hospitalization were less in the local an-esthesia group than in the general anesthesia group(P<0.05).Conclusion Treatment outcomes and safety of balloon di-lation eustachian tuboplasty combined with tympanotomy tube insertion in the treatment of refractory secretory otitis me-dia are similar regardless using local or general anesthesia.Notwithstanding local anesthesia may result in intraoperative awareness and pain reaction,despite its advantages in shorter operative time,fewer adverse events and lower cost.关键词
