Effect of Celecoxib on Cyclooxygenase-2 Expression in Synovial Tissue of Patients with Severe Knee Osteoarthritis After Total Knee Arthroplasty

Abstract
Objective To investigate the effect of celecoxib on the expression of cyclooxygenase-2 ( COX-2 ) in synovial tissue of patients with severe knee osteoarthritis after total knee arthroplasty. Methods Totally 236 patients with severe knee osteoarthritis undergoing total knee arthroplasty from February 2015 to February 2017 in Xinqiao Hospital of Chongqing were selected and divided into the observation group and the control group according to the random number table method, 118 cases in each group. The patients in the observation group were treated with celecoxib for 3 d before operation, and the patients in the control group were not taken. The expression of COX-2 mRNA and prostaglandin E synthase 1 ( mPGEs ) mRNA in synovial tissue were determined by real-time PCR ,and the con centration of COX-2 mRNA protien and mPGEs mRNA protien in synovial tissue and the concentration of PGE2, TNF-α, IL-1β in synovial fluid and serum PGE2 were and compared. Results The expression concentration of COX-2 mRNA in the observation group was ( 5. 94 ± 3. 43 ) × 10-4 mol/L, which was significantly higher than ( 3. 47 ± 6. 14 ) × 10-4 mol/L in the control group ( P < 0. 05 ) . There was no significant difference in the expression of mPGEs mRNA, COX-2 mRNA protein and mPGEs mRNA protein between the two groups ( P > 0. 05 ) . The concentration of PGE2 [ ( 237. 74 ± 51. 97 ) pg/mL ] , TNF-α [ ( 34. 47 ± 13. 74 ) pg/mL ] , IL-1β [ ( 60. 14 ± 51. 16 ) pg/mL ] and serum PGE2 [ ( 284. 11 ± 79. 65 ) pg/mL ] in the joint fluid of the observation group had no significant difference from ( 171. 21 ± 79. 48 ) pg/mL, ( 38. 87 ± 21. 71 ) pg/mL, ( 29. 98 ± 12. 84 ) pg/mL, ( 274. 74 ± 59. 18 ) pg/mL of the control group ( P > 0. 05 ) . Conclusion Taking celecoxib before total knee arthroplasty can inhibit the expression of COX-2 mRNA, but it has no significant effect on the expres-sion of mPGEs mRNA, mPGEs mRNA protein, COX-2 mRNA protein and the concentration of PGE2, TNF-α, IL-1β in the synovial fluid and serum PGE2.关键词
