Abstract
Objective:To evaluate the indications,safety and long-term prognosis of minimally invasive surgery for patients with gastric stromal tumors.Methods:Clinical data of sixty-seven patients of gastric stromal tumor in department of general surgery,the Affil-iated Jinling Hospital,Medical School of Nanjing University from Jan.2008 to May 2016 were analyzed retrospectively.According to the operation method,the patients were divided into the minimally invasive group(n=43)and the laparotomy group(n=24).Clinical pa-thology,perioperative factors,surgical indications and long-term prognosis of the two groups were analyzed.Results:There were no sta-tistically significant differences on sex ratio between the two groups(P=0.55).In the majority of cases,the tumor was located at upper third of the stomach,accounting for 60.47%(26/43)and 58.33%(14/24)in the minimally invasive group and the laparotomy group respectively.The mean diameter of tumor was(4.06±2.28)cm and(3.90±2.82)cm.The intraoperative blood loss of the minimally invasive group and the laparotomy group was(33.61±23.51)ml vs.(137.08±40.24)ml,and the postoperative hospitalization time was(7.00±3.38)d vs.(11.87±3.71)d,and the differences were statistically significant.Patients in the minimally invasive group were not converted to open surgery but one postoperative gastric emptying obstacle was found.All cases were followed up for at least 1 year,and the median follow-up period was 44 months.1 case of recurrence occurred in the minimally invasive group,and no deaths oc-curred during the follow-up period.Conclusions:Minimally invasive technique for the treatment of gastric stromal tumor has advantages of less bleeding and faster recovery.Compared with open surgery,the minimally invasive technique has more advantages and the same postoperative long-term survival effects when tumors locate at esophagogastric junction or pylorus.关键词
