胸椎旁神经阻滞对胸腔镜肺癌根治术后疼痛程度、应激反应、免疫应答的影响
Effect of thoracic paravertebral block on the pain,stress response and immune response af-ter thoracoscopic resection of lung cancer

Abstract
Objective:To study the effect of thoracic paravertebral block on the pain,stress response and immune re-sponse after thoracoscopic resection of lung cancer.Methods:Patients who underwent thoracoscopic resection of lung cancer in Mianyang Central Hospital between March 2015 and March 2017 were selected as the research subjects and randomly divided into the nerve block group who accepted thoracic paravertebral block combined with regular general anesthesia and the normal control group who accepted regular general anesthesia.The levels of pain mediators and stress hormones in serum were meas-ured before surgery as well as 3 days and 6 days after surgery;the contents of immune cells and immune transcription factors in peripheral blood were determined 3 days and 6 days after surgery.Results:3 days and 6 days after surgery,SP,BK,IL-1β, IL-6,Cor,INS,NE and E contents in serum of both groups of patients were higher than those before surgery,and SP,BK, IL-1β,IL-6,Cor,INS,NE and E contents in serum as well as RORγt,Foxp3,PD-1 and PD-L1 expression in peripheral blood of nerve block group were lower than those of control group whereas CD3+CD4+T cell,CD3+CD8+T cell and CD16+CD56+NK cell contents as well as T-bet expression in peripheral blood were significantly higher than those of normal control group.Conclusion:Thoracic paravertebral block can inhibit the pain and stress response,and improve the immune response after tho-racoscopic resection of lung cancer.关键词
