Application of conventional drainage combined with vacuum sealing drainage on the treatment of postoperative wound in recurrent soft tissue sarcoma

Abstract
Objective To evaluate the therapeutic effect of combining vacuum sealing drainage (VSD) with traditional drainage on the repair of postoperative wound caused by the resection of recurrent soft tissue sarcoma.Methods From Sep 2015 to Jun 2017,18 patients with recurrent sarcoma who underwent surgery were included.Wide local excision or radical resection are used to obtain a margin range of surgical boundaries.Meanwhile,the VSD were applied in the postoperative wound of all patients,and porous silica gel drainage tube was conventionally placed inside the wound cavity to realize the external connection with negative pressure drainage device.VSD were removed after 5 ～ 7 days,silica gel drainage tube was removed when the drainage fluid was less than 20 mL/24 h.Clinical data,follow-up observation of surgical time,intraoperative bleeding,drainage volume,postoperative complications,limb function recovery and prognosis of oncology were analyzed.Results 18 cases of recurrent soft tissue sarcoma were completely resected,incision length (13.8±5.0) cm (range:5 ～ 25) cm,operation time (159.7±42) min (range:87 ～230) min,bleeding (568.9±195.4) mL (range:300 ～ 950) mL.The traditional drainage volume was 436.1 ± 189.9 mL (range:100 ～ 800) mL,andtheVSD flow volume was 544.4±163.7 mL (range:260 ～ 800) mL.There was no significant difference in postoperative drainage volume and VSD volume between the two groups (P＞0.05).The mean follow-up of all patients was (28.6±9.3) months (range:11 ～ 47 months).The average MSTS scores (22.2±4.3) (range:16 ～ 28),83.3％ of the excellent and good rate after three months of operation.At the last follow-up,13 patients without recurrence,3 cases oflocal tumor recurrence,and again underwent surgical resection,2 cases of malignant tumor patients with distant metastasis and death.Conclusion Conventional drainage combined with VSD is a an effective and safety method for treatmentof surgical wound in recurrent sarcoma,contributing drainage and eliminate cavity adequately with shorter hospital stays,and greater rates of primary wound closure.But it still to be a large sample and long-term follow-up results to further confirmation.关键词
