Abstract
Objective To investigate the clinical value of β2-microglobulin (β2-MG),Cystatin C(Cys-C),urinary neutrophil gelatin-related apolipoprotein (uNGAL),urinary microalbumin (uMA),Urinary Creatinine (uCr),and Urinary Microalbumin/Creatinine (uMA/Cr) in the diagnosis of early renal function injury in patients with multiple myeloma (MM).Methods A total of 42 MM patients with renal injury treated at our hospital from January,2015 to June,2017 were selected asa study group (MM group) and 45 healthy subjects a healthy control group.The levels of serum β2-MG,serum Cys-C,uNGAL,uMA,uCr,and uMA/Cr in both groups were detected and statistically analyzed.Results The levels of serum β2-MG,serum Cys-C,uNGAL,uMA,and uMA/Cr were higher and the level of uCr was lower in the study group than in the healthy control group (all P < 0.05).In the terms of diagnostic performance (judged by AUC area),uNGAL and uMA/Cr were the best and uCr was the worst;in the terms of diagnostic sensitivity,uMA and uMA/Cr were the best and uCr was the worst;in the terms of diagnostic specificity,β2-MG was the highest,while Cys-C was the lowest;in the terms of screening effect (judged by the youden index),uMA/Cr was the best and uCr was the worst.The positive rates of β2-MG,Cys-C,uNGAL,uMA,uCr,and uMA/Cr in the study group were 71.4%,66.7%,85.7%,54.8%,45.2%,and 88.1%,respectively;and the positive rates of uNGAL and uMA/Cr were the highest.Conclusion The levels of serum β2-MG,serum Cys-C,uNGAL,uMA,uCr,and uMA/Cr closely correlate with the degree of early renal injury in patients with MM.Comprehensive detection of the various indicators above,especially uNGAL and uMA/Cr,is of great significance in the early diagnosis,treatment,and monitoring of MM renal injury.关键词
