Abstract
OBJECTIVE:To investigate the existing problem of the inpatient ' s medication orders, and to probe into the model of clinical pharmacist ' s intervention on the medication orders .METHODS: Clinical pharmacists extracted 1％of the medical orders of discharged patients in each month from Jun .2015 to Jun .2016 in the First People ' s Hospital of Changzhou ( hereinafter referred to as "our hospital") , totally 890 medical orders were included . Retrospective analysis and evaluation was conducted on the rationality of medical orders .RESULTS: Of the 890 medical orders , 389 pieces were irrational , accounting for 43.71％.There were 481 cases with irrational drug use , mainly performed as improper perioperative prophylactic medication (123 cases,25.57％), overdosage (99 cases, 20.58％), improper of administration route (79 cases, 16.42％) and improper selection of solvent (54 cases, 11.23％) .CONCLUSIONS:Only through the clinical pharmacists ' review, intervention and related support work in the hospital, the proportion of rational inpatient's medication orders can be further improved .关键词
