Abstract
Objective:To explore the feasibility,clinical efficacy and risk factors for postoperative complications of laparoscopic left hemihepatectomy for the treatment of recurrent hepatolithiasis.Methods:The retrospective case-control study was adopted.Clinical data were collected from 107 patients who underwent laparoscopic left hemihepatectomy for hepatolithiasis from Jan.1,2008 to Jun.30, 2017.The 107 patients were divided into the observation group with biliary surgical history(n=52) and the control group without bili-ary surgical history (n=55).The surgical related indicators,postoperative complications and efficacy (stone clearance rate and recur-rence rate) of the two groups were comparatively analyzed.Results:Compared with the control group,the duration of operation was sig-nificantly increased in the observation group(P<0.05),and the positive rate of multi-drug resistant bacteria,indicated by intraoperative bile culture,was increased(P<0.05).However,there were no statistically significant differences between the two groups in intraopera-tive blood loss,blood transfusion rate,incidence of postoperative complications or postoperative intestinal function recovery.Multivariate analysis identified preoperative albumin(OR=8.403,95% CI:2.173-32.486,P=0.002) and positive rate of multi-drug resistant bac-teria indicated by bile culture (OR=31.193,95% CI:7.254-134.141,P=0.000) as the risk factors for postoperative complications. Conclusions:Laparoscopic left hemihepatectomy is a safe and effective therapy for recurrent hepatolithiasis.关键词
