Posterior non-fusion occipital-axis screw-rod temporary fixation for treatment of the acute type ⅡC odontoid fractures

摘要

Abstract

Objective To investigate the clinical efficacy of posterior non-fusion occipital-axis screw-rod temporary fixation for the acute type ⅡC odontoid fractures. Methods All of the 24 clinical patients suffered from acute typeⅡC odontoid fractures were operated with primary posterior non-fusion occipital-axis screw-rod temporary fixation. When the fracture was osteounion,the secondary surgery was carried out to remove the internal fixation,the cervical ROM and functional recovery were recorded after the two operations. Results The 24 cases underwent the posterior non-fusion occipital-axis screw-rod temporary fixation successfully. All cases were followed up for 15 ~24 months. The fractures were bony union by CT images after 12~18 months of the primary treatment. The symptoms of feeling and movement disturbance in occipitocervical complex reduced or disappeared, meanwhile the nerve function got a greater degree of recovery. Their spine function of cervical vertebra instantly got partial recovery after the second stage operation. The ROM of cervical vertebra varied at the last follow-up,left and right rotational angle:83.36°± 4.80°, anteversion angle:32.50°± 3.79°, retroversion angle:21.96°± 3.61°. Conclusions For patients with acute typeⅡC odontoid fractures, primary posterior non-fusion occipital-axis screw-rod temporary fixation followed by second stage instruments removed can preserve C1~2joint rotation function furthest.
