两种术式治疗老年肱骨近端骨折的近期疗效

摘要
Abstract
Objective To investigate the short-term clinical effect of open reduction and internal fixation and artificial humeral head replacement for the treatment of proximal humeral fractures in the elderly. Methods The 65 cases of proximal humerus fractures were treated with open reduction and internal fixation(33 cases,fixation group) and arti-ficial humeral head replacement (32 cases, replacement group).At 3 months postoperation and last follow-up, two groups of flexion, abduction and external rotation function were compared; VAS, ASES score and Constant scoring system were used to evaluate the two groups of shoulder function. Results All patients were followed up for 1 year. At 3 months after operation,the forward flexion,abduction,external rotation and the scores of VAS,ASES and Con-stant in the replacement group were significantly better than those in the fixation group,the differences were statisti-cally significant (P<0.001). At the last follow-up, the forward flexion, abduction, external rotation, and VAS, ASES and Constant scores were not significantly different between the two groups(P>0.05). Conclusions Both of open reduction and internal fixation and the artificial humerus head replacement can be used in the treatment of elder-ly patients with proximal humeral fractures,but artificial humerus head replacement can be better pain relief and ear-lier to get better shoulder joint activity.关键词
