Abstract
Objective To observe the clinical outcomes of phacoemulsification and intraocular lens(IOL)implan-tation in patients with angle-closure glaucoma and cataract.Methods 41 patients(44 eyes)with angle-closure glaucoma and cataract treated in Huainan First People's Hospital between March 2013 and March 2017 were enrolled in this study. Inclusion criteria were best corrected visual acuity ≤0.5,fundus C/D≤0.6,dynamic gonioscope angle showing scope of closure≤270 degrees,gonioscope(Scheie)showing the angle narrowing grade III ~IV,Emery lens classification II~III, intraocular pressure(IOP)≤21 mmHg with medication,normal appearance of the pupil.Patients were treated with con-ventional phacoemulsification, IOL implantation, and anterior chamber angle separation.Patients were examined for best corrected visual acuity(BCVA),IOP,central anterior chamber depth,anterior chamber angle structure,corneal endothe-lial cell count,optic nerve OCT,visual field,and complications before and after treatment at 1 day,1 week,l month,3 months and 6 months.Results In 40 cases(43 eyes),the BCVA,IOP,anterior chamber depth, and anterior chamber angle structure significantly improved.Corneal endothelial cell count,optic nerve OCT,visual field did not change signifi-cantly.Corneal edema occurred in 9 eyes,and anterior chamber inflammatory exudate in 6 eyes,both improved within 3~days of treatment.No posterior capsule rupture,vitreous overflow,iris injury or malignant glaucoma.In 1 case(1 eyes), the patient had improved BCVA,deeper anterior chamber angle and open anterior chamber angle but the IOP maintained at 30~40 mmHg.Conclusions Phacoemulsification and IOL implantation combined with anterior chamber angle correction for angle-closure glaucoma patients with cataract is a safe and effective method.关键词
