PCOS患者卵泡液中内脂素、网膜素-1与IVF-ET的相关性研究
Study on the expression of visfatin and omentin-1 in follicular fluid of patients with poly-cystic ovary syndrome ( PCOS) and its relation to IVF-ET

Abstract
Objective:To investigate the effects of visfatin, omentin-1 and metabolic components in follicular fluid on follicular growth,embryo development and pregnancy outcomes in vitro fertilization and embryo transfer ( IVF-ET) in patients with polycystic ovary syndrome ( PCOS) . Methods:130 patients with IVF-ET were selected,including 68 patients with PCOS. The patients were divided into four groups according to BMI:normal weight PCOS group ( n=41),normal weight control group (n=33),overweight PCOS group (n=27) and overweight control group (n=29). Visfatin、omentin-1 and lipid profile (TC、TG、HDL、LDH) in follicular fulid were measured,and correlation with outcome in IVF were analysed. Results:The concen-trations of visfatin and TC in follicular fuild were significantly higher in PCOS than controls ( P<0 . 05 ) . The concentrations of omentin-1 and HDL in follicular fuild were significantly lower in PCOS than controls(P<0. 05). In normal weight PCOS group,the number of oocytes retrieved, MII oocytes number,the concentrations of visfatin,TC,TG and LDL in follicular fuild of were significantly higher than those in normal weight control group, the concentration of omentin-1 and HDL in follicular fuild were significantly lower than those in normal weight control group, the difference had statistical significance(P<0. 05). The concentration of visfatin,TC,TG and LDL in follicular fuild in the overweight PCOS group were significantly higher than those in o-verweight control group,the concentration of omentin-1 and HDL in follicular fuild were signifi-cantly lower than those in overweight control group,the difference had statistical significance( P<0 . 05 ) . The concentration of visfatin in follicular fluid of PCOS showed a positive correlation with BMI(r=0. 645,P<0. 05),the number of oocytes retrieved(r=0. 546,P<0. 05) and the number of high-quality embryos(r=0. 479,P<0. 05). The concentration of omentin in follicular fluid of patients with PCOS showed a negative correlation with BMI(r=-0. 667,P<0. 05),LDL ( r=-0 . 498 , P<0 . 05 ) , and positive correlation with HDL in follicular fluid ( r=0 . 582 , P<0 . 05 ) . Conclusion:The level of visfatin increased and omentin-1 decreased in follicular fluid of patients with PCOS,which indicated visfatin and omentin-1 could be a potential biomarker for PCOS. That regional microenviroment visfatin administration in patients with PCOS could im-prove outcome of COH in IVF pregnancy.关键词
