Reference Values for the Diagnosis of Wrist Joint Instability in Patients with Carpal Instability Using 3.0T Magnetic Resonance Imaging

Abstract
Objective We aimed to investigate the value of 3.0T magnetic resonance imaging (MRI) measurements in patients with carpal instability who suffered proximal row ligament injury of the wrist joint angle. Methods This study enrolled 120 patients (study group) who underwent arthroscopic examination and were initially diagnosed with carpal instability at Harrison International Heping Hospital from January 2015 to January 2017. Thirty healthy volunteers were recruited as the control group at the same time. High field intensity 3.0T MRI scans were performed in the two groups to measure the length, width, and thickness of the scapholunate and triangular ligaments, the angle between the scapholunate and triangular ligaments (scapholunate angle), the angle between the skull and the lunotriquetral (skull angle), and the angle between the radius and the lunotriquetral(radial angle). We compared the signal strengths for the scapholunate and the triangular ligaments in MRI scans of the wrist joints in the two groups. Results The dorsal, palmar, and proximal parts of the scapholunate and triangular ligaments were all longer in the study group compared with the control group (P<0.05). Furthermore, the dorsal, palmar, and proximal parts of the scapholunate and triangular ligaments were all wider in the study group compared with the control group (P< 0.05), and the dorsal, palmar, and proximal parts of the scapholunate and triangular ligaments were thicker in the study group compared with the control group (P<0.05).The scapholunate, skull, and radial angles were all higher in the study group compared with the control group (P<0.05).The scapholunate and triangular ligaments had low signals in the control group compared with the study group.Conclusion 3.0T MRI measurements demonstrate good clinical value in terms of diagnosing carpal instability in patients with proximal row carpal ligament injury of the wrist joint angle. This technique can not only measure the signal intensity of MRI instability of the wrist (scapholunate and triangular ligament), but can also measure the sizes of the scapholunate and triangular ligaments, and the angle between the carpal and radius. 3.0T MRI can thus provide clinical reference values and an objective basis for hand surgeons to determine suitable treatments.关键词
