急性失代偿性心力衰竭患者血清碱性磷酸酶与肾功能恶化的相关性研究
Correlation between Serum Alkaline Phosphatase and Worsening Renal Function in Patients with Acute Decompensated Heart Failure

Abstract
Objective The purpose of this study was to determine the correlation between serum alkaline phosphatase (ALP) and worsening renal function (WRF) in patients with acute decompensated heart failure (ADHF). Methods We enrolled 972 consecutive ADHF patients who were treated in Cardiovascular Department or Geriatrics Department, Hainan General Hospital from June 2013 to June 2016. We assigned 840 of whom with WRF and 132 without to WRF group and non-WRF group, respectively, and compared their baseline characteristics, laboratory findings, echocardiographic results. Meanwhile, we classified these 972 cases into tertiles by ALP level (<203 U/L, 203-278 U/L and >278 U/L) and compared the incidence of WRF between them. Logistic regression analysis was performed to investigate the factors associated with the incidence of WRF. Results The distribution of sex, prevalence of diabetes, prevalence of chronic kidney disease, history of treatment with furosemide injection, norepinephrine, angiotensin converting enzyme inhibitor or angiotensin Ⅱ receptor blocker and furosemide tablets, levels of creatinine, glomerular filtration rate, hemoglobin, albumin, total bilirubin, and alanine transaminase differed significantly between WRF group and non-WRF group (P<0.05). The incidence of WRF increased with the growth of ALP (χ2trend=8.827,P=0.004). Logistic regression analysis identified that chronic kidney disease, diabetes and, serum albumin and ALP, were associated with the incidence of WRF (P<0.05). Conclusion Serum ALP is an independent predictor of WRF in the clinical course of ADHF.关键词
