社区慢性精神分裂症患者家庭照护者认知团体心理治疗的近期与远期干预效果研究
Short-term Effects and Long-term Effects of Group Psychotherapy on Family Caregivers of Patients with Chronic Schizophrenia in Community

Abstract
Objective To investigate the short-term effects and long-term effects of group psychotherapy on family caregivers of patients with chronic schizophrenia in community. Methods A total of 80 patients with chronic schizophrenia and one main family caregiver of each of them were selected in Hudong community health service center of Pudong New Area in Shanghai from January to February in 2016. The 80 family caregivers were randomly divided into the control group (n=40) and the observation group (n=40). Routine follow-up and health education were given to the control group while additional group psychotherapy was given to the observation group besides routine follow-up and health education. Interventions in these two groups lasted for 8 weeks. The mental status of family caregivers were evaluated by Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), Life Satisfaction Index A Scale (LSIA), Family Burden Scale (FBS), Social Support Rating Scale (SSRS) and Relative Stress Scale (RSS) at baseline (week 0), completion time of intervention (week 8) and 6 months after intervention. Results The scores of SAS, SDS and LSIA were significantly different not only between two groups but also at different time points (P<0.05), while there was an interaction effect among groups and time points on SAS, SDS and LSIA scores (P<0.05). In addition, the scores of SAS, SDS and LSIA were significantly different between two groups at week 8 and 6 months after intervention (P<0.05). The FBS scores of the impact on family daily activities and the impact on family members' physical health were significantly different not only between two groups but also at different time points (P<0.05), while there was an interaction effect among groups and time points on the total FBS scores as well as the subscores of the impact on family daily activities, the impact on family relationships, and the impact on family members' physical health (P<0.05). In addition, the subscores of the impact on family daily activities and the impact on family members' physical health were significantly different between two groups at 6 months after intervention (P<0.05). The total scores and subscores of SSRS were significantly different not only between two groups but also at different time points (P<0.05), while there was an interaction effect among groups and time points (P<0.05). In addition, the total scores and subscores of SSRS were significantly different between two groups at 6 months after intervention (P<0.05), while only total scores and subscores of subjective support and support utilization were significantly different at week 8 (P<0.05). The total scores and subscores of RSS on psychological distress and negative emotions due to patient caring were significantly different not only between two groups but also at different time points (P<0.05). There was an interaction effect among groups and time points on the total scores and subscores of RSS (P<0.05).In addition, the total scores and subscores of RSS were significantly different between two groups at 6 months after intervention (P<0.05). Conclusion The short-term effects of group psychotherapy on family caregivers of patients with chronic schizophrenia are reducing anxiety and depression,improving life satisfaction and providing effective social support, while besides these effects the long-term effects also include decreasing family burden and relieving the stress level.关键词
