新生儿CM V感染情况及其与连接蛋白Cx26基因突变的相关性
Neonatal cytomegalovirus infection and its association with connexin Cx 26 gene mutations

Abstract
Objective To investigate the cytomegalovirus(CMV)infection in neonates and its relationship with the mutation of connexin Cx26 gene.Methods A total of 104 newborns admitted in Guangzhou Women's and Children's Medical Center from May 2014 to February 2017 were selected as research objects,including 42 cases with negative CMV-DNA assigned into group A and 62 cases with positive CMV-DNA assigned into group B.Umbilical cord blood was collected in two groups.Cx26 mRNA expression was detected with reverse transcription polymerase chain reaction(RT-PCR)in two groups.Hearing status of infants was followed up and blood biochemical examination was done in two groups.Hearing test results,correlation between Cx26 gene mutations and CMV infection were analyzed.Results No neonate in group A showed abnormal blood biochemical indexes,while 27 cases in group B had congenital CMV infection with abnormal liver function and other 35 cases had congenital CMV infection with normal liver function.Total bilirubin(TBil),aspartate aminotransferase(AST)and alanine transaminase(ALT)in group B were higher than those in group A,and differences were statistically significant(t value was 9.57,15.73 and 12.04 respectively,all P< 0.05). Thirty-six cases passed initial hearing screening,and remaining 6 cases passed second hearing screening in group A.Twenty-four cases passed initial hearing screening and 4 cases passed second hearing screening in neonates with normal liver function in group B. In other 7 cases with normal liver function in group B,3 cases were detected with abnormal hearing by brainstem auditory evoked potential examination.Eight cases passed initial hearing screening and 6 cases passed second hearing screening in neonates with abnormal liver function in group B.In other 13 cases with abnormal liver function in group B,8 cases were detected with abnormal hearing by brainstem auditory evoked potential examination.Partial correlation analysis with sensorineural hearing loss variable controlled showed that correlation coefficient of Cx26 gene mutation and CMV infection was 0.571(P<0.05).Partial correlation analysis with CMV infection variables controlled showed that correlation coefficient of sensorineural hearing loss and Cx 26 gene mutations was 0.209(P<0.05).Partial correlation analysis with Cx26 gene mutation variable controlled showed that correlation coefficient of sensorineural deafness and CMV infection was 0.381(P< 0.05).Conclusion Neonatal CMV infection will cause mutations in Cx26 gene,and will further damage the hearing of children.Abnormal liver function type CMV infection will greatly increase probability of sensorineural deafness and Cx26 gene mutation in patients.关键词
