Abstract
Objective To investigate the effect of combined spinal epidural anesthesia in pregnancy-induced hypertension syndrome pregnant women in cesarean section.Methods A total of 86 puerperae with gestational hypertension were selected as research objects and were divided into control group(n=45)and observation group(n=41)according to random data table method.The patients in both two groups were given conventional treatment such as dilation,decompression and sedation.Based on this treatment,the patients in the control group were treated with continuous epidural anesthesia,while the patients in the observation group were treated with combined spinal-epidural anesthesia.The anesthesia onset time,the time between onset of anesthesia and delivery,operation time,the levels of HR,DBP and SBP at T1,T2,T3 and T4,umbilical cord blood gas parameters,the Apgar scores at 1 min and 5 min after delivery and the occurrence of adverse reactions between two groups were compared.Results The anesthesia onset time,the time between onset of anesthesia and delivery as well as the operation time of patients in the observation group were significantly shorter than those of patients in the control group with statistically significant differences(t value was 10.163,9.754 and 7.632,respectively,all P<0.05).There were no significant differences in the levels of HR,DBP and SBP at T1 and T2 between two groups(t value ranged 0.016—1.651,all P>0.05).At T3,the HR level of patients in the observation group was significantly lower but the levels of DBP and SBP were significantly higher than those of patients in the control group,and at T4 the levels of HR and SBP of patients in the observation group were significantly higher than those of patients in the control group with statistically significant differences(t value ranged 2.207—6.051,all P<0.05).There was no significant difference in PaCO2,PaO2and pH levels and the Apgar scores at 1 min and 5 min after delivery between two groups(t value ranged 0.233—1.941,all P>0.05).The occurrence of adverse reactions in the observation group was significantly lower than that in the control group with statistically significant difference(χ2=8.882,P<0.05).Conclusion The combined spinal-epidural anesthesia is of good anesthetic effect on cesarean section for puerperae with gestational hypertension,which can stabilize blood pressure and heart rate. It is very safe and worthy of clinical promotion.关键词
