一氧化氮吸入联合高频震荡通气对严重低氧血症新生儿预后的影响
Effects of inhaled nitric oxide combined with high frequency oscillatory ventilation on prognosis in neonates with severe hypoxemia

Abstract
Objective To investigate the effect of inhaled nitric oxide (iNO) combined with high frequency oscillatory venti-lation (HFOV) on prognosis and neurodevelopment in neonates with severe hypoxemia .Methods 150 cases of severe hypoxemia in our hospital from March 2015 to January 2017 were enrolled in this study ,all the children were divided into observation group (n=75) and control group (n=75) according to the order of admission .The children in observation group were treated with HFOV combined with iNO ,patients in the control group were treated with HFOV .The curative effects of the 2 groups were com-pared .The blood gas index ,nerve development and prognosis of the 2 groups were measured and compared .Results The effective rate of the observation group was significantly higher than that of the control group (P<0.05) .24 h to 48 h after treatment ,the PaO2 levels of 2 groups showed increasing trend ,the level of PaCO2 decreased gradually (P<0.05 or P<0.01) ,and the differ-ence between the 2 groups was significant (P<0.01) ,but the pH value between groups and before and after treatment showed no significant difference (P>0.05) .The oxygen treatment time and the mechanical ventilation time of the observation group were significantly shorter than those of the control group (P<0.05 or P<0.01) ,however ,there was no significant difference in gas leakage ,pulmonary hemorrhage and severe intracranial hemorrhage between the 2 groups (P>0.05) .The 2 groups were followed up for 6 months after discharge ,the follow-up rate was 100% ,the results showed that there was no significant difference in prog-nosis between the 2 groups (P>0.05) .After treatment ,the scores of behavior and active muscle tone in the observation group were significantly higher than those in the control group (P<0.05) ,but general condition ,passive muscle tension and original re-flex scores were no difference between the 2 groups (P>0.05) .Conclusion The combination of HFOV and iNO can improve the clinical symptoms and oxygenation status of children with severe hypoxemia ,and has no significant effect on neurodevelopment and prognosis ,it is worthy of clinical application .关键词
