摘要
Abstract
In order to integrate the regional medical resources and implement the hierarchical diagnosis and treatment system,a new "four stops" rehabilitation service mode was established based on the rehabilitation medical union of Huadong Hospital and Jing'an District which includes the second and third grade hospitals,community health service centers,community health service stations and families to ensure patients can get the standardized,effective and continuous rehabilitation services. This paper introduces the concept and framework of the new mode,and analyzes its advantages,such as the two-way referral mechanism,drug delivery mechanism,resource sharing and the quality control assessment system in order to give some suggestions for the future development and existing problems of the rehabilitation medical union.关键词
