Abstract
Objective To investigate the effects of parecoxib sodium combined with dezocine on remifentanil-induced hyperalgesia in patients with gastric cancer after laparoscopic surgery.Methods A total of 160 patients with gastric cancer undergoing elective laparoscopic surgery were randomly divided into blank control group (group A),simplex dezocine group (group B),simplex parecoxib sodium group (group C),and parecoxib sodium combined with dezocine group(group D),with 40 cases in each group.After preoperative anesthesia induction,group B was treated with intravenous injection of dezocine(0.1 mg/kg),group C with parecoxib sodium(0.2 mg/kg),group D with dezocine(0.1 mg/kg) + parecoxib sodium(0.2 rng/kg),and group A with the equivalent volume of normal saline.Mean arterial pressure(MAP),HR and Visual Analogue Scale(VAS) score were compared among the four groups before induction(T0),at the moment of extubation(T,),after one hour of extubation (T2),after 3 hours of operation(T3),after 6 hours of extubation (T4) and after 12 hours of extubation (T5),respectively,and the incidence of adverse reactions was compared as well.Results Significant differences were found in the MAP,HR and VAS score in the four groups at different time points(P ＜ 0.05),MAP,HR and VAS score changed over time in the four groups (P ＜ 0.05),and there was interaction effects between group and time(P ＜ 0.05).The result of one-way ANOVA showed that MAP,HR and VAS score in group A were higher than those in group B,group C or group D at T1-T5 (P ＜ 0.05),MAP,HR and VAS score in group B and group C were higher than those in group D at Tt-T5 (P ＜ 0.05),and there was no statistically significant difference in the MAP,HR or VAS score in group B or group C at T1-T5 (P ＞ 0.05).No significant difference was observed in the incidence rate of postoperative adverse reactions among the four groups (P ＞ 0.05).Conclusion Combined use of parecoxib sodium and dezocine has better efficacy for reducing remifentanil-induced hyperalgesia in patients with gastric cancer after laparoscopic surgery compared to either of them,and has less impacts on hemodynamics.关键词
