分期手术治疗巨大垂体腺瘤临床研究
Clinical analysis of staged surgery for giant pituitary adenomas
摘要
Abstract
Objective To explore the operation methods and the clinical significance of staging operation in giant pituitary adenomas.Methods The clinical data of 18 patients with giant pituitary adenomas who underwent the surgery by stages were analyzed retrospectively.According to the different morphology and growth pattern of the tumor,we use two stage operation strategy.The first stage surgery via nasal-transsphenoidal approach for the sphenoid sinus and intrasellar tumors was performed in 2 patients,in whom the second stage surgery via the subfrontal approach for the suprasellar and intraventricular tumors was performed.On the other hand,the first stage surgery via the subfrontal approach for the suprasellar and intraventricular tumors was performed in 16 patients,in whom the second stage surgery via nasal-transsphenoidal approach for the intrasellar tumors was performed.Results Total removal of the lesion was achieved in 13 cases,3 patients underwent subtotal resection,and 2 patients underwent partial removal.There were no deaths and serious complications after the operation.The postsurgical follow-up period varied from 2 months to 3 years.The improvement of 18 patients had different degrees of clinical symptoms.Patients with drugs or gamma knife treatment of residual tumor,tumor recurrence was not found.Conclusion For giant pituitary adenomas,adopt different approaches and staged surgical strategy to improving the total resection rate according to tumor characteristics and the principle of individualization.During operation,attention should be paid to the protection of the pituitary stalk and hypothalamus,which can reduce postoperative complications and reduce the risk of operation,and can achieve satisfactory results.关键词
