Comparisons of clinical outcome between GnRH antagonist protocol and GnRH agonist long protocol in patients with different stages of endometriosis

Abstract
Objective:To compare IVF outcomes of GnRH agonist(GnRH-a)and GnRH-antagonist(GnRH-ant) protocols in endometriosis patients who had undergone laparoscopy or trans-abdominal operation.Methods:A total of 583 patients undergone IVF/ICSI treatment in the reproductive center of Jiangsu Province Hospital between January 1,2014 and June 30,2016 were included in this retrospective study.The patients had been diagnosed as endometriosis by laparoscopic or trans-abdominal operation.The patients were divided into three groups according to the ovarian stimulation protocols:ultra-long GnRH-a,long short-acting GnRH-a and GnRH-ant groups.The clinical and laboratory parameters of different protocols were compared and analyzed.Results:There were no significant differences in clinical pregnancy rate,live birth rate and implantation rate by using three different protocols in women with mild(stages Ⅰ-Ⅱ)and severe(stages Ⅲ-Ⅳ)endometriosis(P＞0.05).The gonadotrophin(Gn)doses and stimulation days were significantly higher in ultra-long GnRH-a protocol than the other two protocols(P＜0.05).The fertility rate and good quality embryo rate tended to be higher in GnRH-ant protocol than GnRH-a protocol for the patients with stages Ⅲ-Ⅳ endometriosis (P ＜ 0.05).The number of oocytes retrieved was significantly lower in severe endometriosis patients compared with women with mild endometriosis.Conclusions:The GnRH-ant protocol can be used to induce ovulation in patients with different stages of endometriosis,which can achieve similar IVF outcomes as long GnRH agonist protocol and reduce the cost and time of drug treatment.关键词
