Abstract

Objective To investigate the effects of Glibenclamide combined with insulin on bone mineral density, blood rheology and coagulation function in patients withgestational diabetes mellitus. Method From January 2016 to September 2017, 82 cases of gestational diabetes mellitus in our hospital were selected as the subjects. According to the random table method, they were divided into observation group and control group, 41 cases in each group. Control group patients were treated with insulin, and observation group patients were treated with Glibenclamide on the basis of control group. The course of treatment in the two groups was 8 weeks. The curative effects, the changes of glucose metabolism, SI, hemorheology and coagulation function before and after treatment, and the outcome of pregnancybetween the two groups were compared. Result The total effective rate of treatment in observation group was higher than that of control group (P < 0.05). After treatment, the levels of fasting plasma glucose (FPG), 2-hour postprandial plasma glucose (2hPG) and glycated hemoglobin A1c (HbA1c) decreased in the two groups (P < 0.05); the levels of FPG, 2hPG and HbA1cin observation group were lower than control group (P<0.05). After treatment, SI of the two groups were significautly incleased (P < 0.05) in observation group was higher than that of control group (P < 0.05). The whole blood viscosity, plasma viscosity, low shear and high shear whole blood viscosity decreased in the two groups after treatment (P<0.05); the above indexes of observation group after treatment were significant lower than control group (P<0.05). After treatment, the prothrombin time (PT) and activited partial thomboplastin time (APTT) increased and fibrinogen (Fib) decreased in the two groups (P<0.05); the PT and APTT in observation group were significantly longer than those in control group (P<0.05), and the level of Fib was significantly lower than that of control group (P<0.05). The rate of premature delivery in observation group was significantly lower than control group, but there was no significant difference between the two groups (P>0.05). The proportion of neonatal distress, preeclampsia and neonatal hyperbilirubinemia in observation group was significantly lower than that in control group, and there were significant differences between the two groups (P<0.05). Conclusion Glibenclamide combined with insulin is effective in the treatment of gestational diabetes mellitus. It can increase bone mineral density, improve the coagulation function, hemorheology and pregnancy outcome. It has important clinical research value.
