A study on the extended margin of planning target volume of intensity modulate radiotherapy for head and neck neoplasm base on CBCT

摘要

Abstract

Objective: To provide reference data for the extended margins of clinical target volume(CTV) and planning target volume(PTV) by using cone-beam computed tomography(CBCT) to measure and analyze setup error of intensity modulate radiotherapy for head and neck neoplasm. Methods: 21 patients with head and neck neoplasm who received intensity modulate radiotherapy were randomly enrolled in this study. All of patients were immobilized with thermoplastic device, and the on -board imaging system of Varian TrueBeam linear accelerator was used to implement scan of CBCT before they received radiotherapy. After the image was acquired, the setup errors on three directions included right-left(X), anterior-posterior(Y) and up-down(Z) were obtained through compared with location of CT. Besides, the average setup errors of radiotherapy between the first 4 weeks and the 5th-7th weeks was compared. Results: Total of 136 images were obtained from 21 patients, and the setup errors of X, Y and Z were(-0.39±1.33)mm, (-0.29±1.38)mm and (-0.32±1.46)mm, respectively. And the difference of setup errors between the first 4 weeks and the 5th-7th was no significant(Z=-1.05, Z=-2.11, Z=-1.88, P<0.05). Conclusion: The change of setup errors on head and neck neoplasm at the later period of radiotherapy was no obvious. Therefore, the research suggests the extended margins of PTV should be 3.55mm, 3.25 mm and 3.08 mm on the three directions of X, Y and Z, respectively.
