Abstract
Objective To understand diet barriers in pregnant women with gestational diabetes mellitus(GDM)and their association with glycaemic control.Methods Questionnaire survey was conducted among 234 return visit pregnant women with GDM who had received health education and individualized nutritional regimen in nutrition clinic of Beijing Obstetrics and Gynecology Hospitals, Capital Medical University from March to June 2017,and the contents of questionnaire included general information and diet barriers.The patients were divided into study group(unsatisfied glycaemic control)and control group(satisfied glycaemic control) for analysis.Results GDM patients in the study group had higher pre-pregnancy BMI,fasting glucose and 1h postprandial glucose of OGTT than the control group(t value was 4.133,6.839 and 2.178,respectively,all P<0.05).The proportion of patients"not willing to be deprived from freedom of food selection"was higher in the study group than in the control group(χ2=4.394,P<0.05).The most common item of diet barriers was"eating outside"both in the study and the control group.After adjustment for all confounding factors including age,nationality,educational level,profession and history of diabetes mellitus,multivariate Logistic regression analysis showed that monthly family income under RMB 5 000,pre-pregnancy BMI,fasting glucose of OGTT and "not willing to be deprived from freedom of food selection"were independent risk factors of poor glycaemic control.Conclusion As one of the independent risk factors of poor glycaemic control,diet barriers should be assessed in clinics for early detection and targeted interventions.关键词
