右美托咪定镇静对机械通气脓毒症患者肠屏障功能的影响
Influence of dexmedetomidine on intestinal barrier function in sepsis patients with mechanical ventilation

Abstract
Objective To analyze the effects of dexmedetomidine (Dex) on intestinal barrier function in mechanical ventilation septic patients.Methods Ninety intensive care unit (ICU) mechanical ventilation septic patients were randomly divided into treatment group (45 cases) and control group(45 cases).Treatment group was given intravenous pumping 1 μg · kg-1 dexmedetomidine for 10 min,and followed by 0.2-0.8 μg · kg-1 · h-1.Control group was intravenously given 0.1 mg·kg-1 midazolam for 30 s,and followed by 0.03-0.15 mg · kg-1 · h-1 intravenous pumping.The levels of D-lactat,endotoxin between the two groups were compared.The ICU hospitalization time,the morbidity of multiple organ dysfunction syndrome (MODS),the patients' Sequential organ failure assessment (SOFA) score were compared.Results At 7 d after treatment,the levels of endotoxin and D-lactat in control group were (10.15 ±2.08) U · L-1,(4.94 ±1.96) mg · L-1,had significant difference with those in treatment group,which were (9.31±1.48) U · L-1,(4.01 ±1.36) mg· L-1(P＜0.05).The rates of MODS in treatment group and control group were 11.11％,28.89％ on 7 d after treatment (P ＜0.05).There was no significant difference between treatment group and control group in SOFA score (P ＞ 0.05).There were no adverse drug reactions in treatment group,there was one case of bradycardia in control group,with no significant difference (P ＞ 0.05).Conclusion Dexmedetomidine can protect intestinal barrier function of the mechanical ventilation soptic patients,and can reduce the rate of MODS.关键词
