骨填充网袋成形术治疗后壁破损的骨质疏松性椎体骨折疗效观察

Abstract
Objective To observe the effect of vesselplasty in treating complicated osteoporotic vertebral compression fractures(OVCFs) with posterior wall of vertebral body deficiency. Methods The 35 patients (total 37 vertebrae) of complicated OVCFs combined with posterior wall of vertebral body deficiency were treated with vesselplasty. Bone cement leakage was observed. Pre-and post-operative VAS,Oswestry dability index(ODI),vertebral anterior height rate and sagittal kyphosis Cobb angles were compared. Results The 35 patients were all followed up for 12 ~15 months. Only one vertebra of bone cement leakage happened in the front vertebra and two vertebrae of bone cement leakage happened in the lateral vertebra. There was no bone cement leakage happened in the posterior vertebra and intraspinal. VAS were significantly reduced from 8.0 ± 1.7 preoperatively to 2.3 ± 0.6 at 2 d after operation, and 2.0 ± 0.8 at the last follow-up. ODI were significantly reduced from 73.4% ± 4.6% to 23.7% ± 3.6% at 2 d after operation,then to 22.5% ± 4.2% at last follow-up. Kyphosis Cobb angle were significantly reduced from 27.3° ± 6.8° to 15.5°± 4.9° at 2 d after operation, then to 16.1° ± 5.2° at last follow-up. Vertebral anterior height rate were significantly improved from 62.7% ± 20.4% to 78.9% ± 18.3% at 2 d after operation, then to 74.8% ± 16.9% at last follow-up. There were statistical differences between the preoperative data and the data at 2 d after op-eration and final follow-up in those above indexes(P<0.05). Conclusions Vesselplasty for treatment of complicat-ed OVCFs with posterior wall of vertebrae deficiency can relieve pain and improve activity effectively,as well as con-trol bone cement leakage.关键词
