Ultrasonic diagnosis of lung blast injury and its consistence with CT diagnosis

Abstract

Objective To evaluate the application value of ultrasonic diagnosis of lung blast injury.Methods A total of 30 rabbit models with lung blast injury were established.The anterior chest wall was divided into four lung zones,all subject to ultrasonic and high-resolution CT (HRCT) examinations.The following ultrasonograms were observed and recorded:(1)Each intercostal space was scanned and the lung zone implicated by more than 4 comet tail signs on the same section and the number of such lung zones;(2)the position where pleural effusion occurred was recorded;(3)whether pneumothorax occurred and the position of pneumothorax.The consistence of ultrasonic method with HRCT diagnosis of lung blast injury and the accuracy of diagnosis of lung blast injury with the ultrasonic method were analyzed.Results Among the 30 experimental rabbits with lung blast injury,the ultrasonic examination confirmed the positive rates of 13.3% for the occurrence of over four comet tail signs and lung zones on the same section (one lung zone was implicated in four cases),30.0% (two lung zones were implicated in nine cases),36.7% (three lung zones were implicated in 11 cases) and 20.0% (four lung zones were implicated in six cases),respectively.The positive rates for lung ground-glass diffuse change corresponding to HRCT were 13.3%,26.7%,40.0% and 20.0%,respectively.There were no statistical differences between the two aspects (P > 0.05).The positive rates for pleural effusion and pneumothorax detected with the ultrasonic method were 23.3% and 10.0%,respectively,showing no statistical differences from the positive rates (23.3% and 13.3%) with the HRCT method (P > 0.05).Taking the HRCT as the golden standard for diagnosing lung blast injury,the overall diagnostic accuracy of the ultrasonic method in diagnosing lung disease of 30 experimental rabbits was 90.0% (27/30).Conclusion Ultrasonic diagnosis of lung blast injury is consistent with and as accurate as the HRCT method,which may be applied as a preliminary means of examining lung blast injury.
