Abstract
Objective To analyze the impacts of two surgical methods (i.e.,intraperitoneal approach and retroperitoneal approach) on body hemodynamics and stress response levels.Methods A total of 94 patients scheduled for laparoscopic intraperitoneal and retroperitoneal approach procedures were selected as the subjects for the research and were divided into two groups:the intraperitoneal approach group (n =38) and the retroporitoneal approach group (n=56).The changes in hemodynamic indicators,including systolic pressure,heart rate (HR),mean arterial pressure (MAP),xyhemoglobin saturation (SpO2) and end-tidal carbon dioxide partial pressure (EtPCO2),were monitored during the operation,and the body temperature,C-reactive protein (CRP),interleukin-6 (IL-6),blood glucose,cortisol and other stress response indicators were recorded before the operation,upon the operation completion,and two weeks after the operation.Results At all the time nodes after the establishment of pneumoperitoneum,the systolic pressure and EtPCO2 of both groups firstly ascended and then descended (P < 0.05) while MPA and SpO2 showed no significant changes (P >0.05).Besides,at 10min and 30min after the establishment of pneumoperitoneum,the intraperitoneal approach group exhibited better systolic pressure,HR and EtPCO2 than the retroperitoneal approach group (P < 0.05).For both groups,blood glucose and cortisol ascended greatly after the operation;CRP and IL-6 also increased significantly two days after the operation (P < 0.05) while CRP and IL-6 levels of the retroperitoneal approach group were significantly lower than the intraperitoneal approach group (P < 0.05).Conclusion In comparison with the intraperitoneal approach,the retroperitoneal approach procedure has less impact on body hemodynamics and thus can avoid unnecessary damage,produce minor stress response and enable the patient to recover from the operation quickly.关键词
