Does Intra-cochlear Infusion After Cochleostomy for Cochlear Implantation Affect Neural Response Telemetry Thresholds

Abstract
Objective We aimed to compare neural response telemetry(NRT)thresholds in children receiving co-chlear implants with or without intra-cochlear dexamethasone(DXM)or hyaluronic acid(HA)infusion upon cochleosto-my as priming substances.Methods This was a hospital based study of pediatric cochlear implant(CI)patients during the period between June 2015 and January 2016 at Changgeng Memorial Hospital(CGMH),Xiamen,China.The NRT software was administered to 75 operated ears in 73 children(mean age at implantation:4.1 years)via pre-curved Co-chlear Nucleus electrodes.The patients were divided in to 3 groups based on intra-cochlear infusion:i.e.DXM(Group DXM,n=26),HA(Group HA,n=20)and no priming agents infusion(Group N,n=29).Five intra-cochlear elec-trodes (number 1 and 6 for high frequency, number 11 for mid frequency, and number 16 and 22 for low frequency) were tested intraoperatively and at initial device activation at two weeks post-operatively.One-way analysis of variance (ANOVA)was used to compare the number of activated electrodes and mean NRT thresholds in the 3 groups.Paired t test and Wilcoxon test were applied to analyze NRT thresholds changes from intraoperative testing and initial activation. Differences in the rate of no-response low and high frequency electrodes among the 3 groups were tested by Pearson χ2 test.Results There was no statistically significant difference in the mean NRT threshold among the 3 groups intraopera-tively or at device activation(P=0.13 and 0.77 respectively),although thresholds for basal turn electrodes in the DXM group were lower.In all 3 groups,the intraoperative mean NRT threshold was higher than that at initial device activa-tion.Conclusions The rate of no-response electrodes during operation and at initial switch-on was similar with or with-out dexamethasone/hyaluronic acid priming.High frequency electrodes at basal turn,when primed with dexamethasone, appear to yield lower response thresholds.关键词
